About this ranking
About this metric. “Crude Death Rate by Country (Time Series)” reformats data published by the World Bank (World Bank indicator code SP.DYN.CDRT.IN). Values are expressed in per 1,000 people.
What to watch. Watch the top of the table completely change meaning: in 1960 the highest crude death rates belonged to poor countries like Nigeria and Chad; today they belong to wealthy, long-lived, ageing societies such as Monaco and Bulgaria. Same number, opposite story.
Caveats. The crude death rate reflects age structure more than health: an ageing population pushes it up even with excellent healthcare, which is exactly why Monaco ranks high. Use age-standardised mortality or life expectancy to compare actual health outcomes.
Trends in the data
This indicator carries a trap, and it is best seen through individual countries. Chad recorded 25.57 deaths per 1,000 in 1960, second in the world; by 2024 it is 11.08, less than half. Vaccines, oral rehydration therapy and safe water cut infant mortality sharply, and most of the high-mortality countries of that era followed the same path. Bulgaria moved the opposite way, from 8.1 per 1,000 in 1960 to 15.6 — almost double. Health care did not deteriorate. Falling births and the emigration of young adults aged the population rapidly, and the age structure of the denominator pushed the rate up.
Japan embodies that second movement most dramatically. It ranked 27th among the countries covered in 1960; by 2024 it has risen to 9th — while its health care remains among the best in the world and life expectancy keeps rising. The fastest ageing on earth has steadily pushed Japan towards the “high mortality” end of this statistic. Bulgaria (24th → 2nd) and Serbia (21st → 3rd) show the same movement, while Chad fell from 2nd to 26th and Lesotho from 3rd to 29th. Over 64 years the top and bottom of the table have essentially swapped places.
Look at the bottom of the latest year and the pattern repeats in reverse. Gulf states and young countries in Africa and South Asia sit there. Where migrant workers and other young adults make up most of the population, deaths per head of population stay low regardless of how ordinary the health system is. Both ends of this ranking are, in other words, a map of age structure. Long-lived countries including Japan will keep moving up it as they age further.
One year deserves a pause: Bosnia and Herzegovina leads 1992 at 19.57 per 1,000. That is a figure from the middle of a civil war, and it records quietly that a crude death rate, normally a statistic about age structure, becomes literally a statistic about death in years of war or catastrophe. The temporary rises visible in many countries during 2020–2021 are the same phenomenon. To compare health itself, use life expectancy; to check the population structure behind the number, use the share of population aged 65 and over. A crude death rate is safest read alongside both.
